Kindergarten Registration for the 2024-25 school year:
- We are 2 weeks away from KG screening for the 24/25 school year. Please registered your child and/or complete all required steps ASAP.
- Registration for Kindergarten at Bath Elementary has begun! To be eligible, a child must be 5 years old on or before August 1, 2024. If you have a child who meets this qualification:
- Visit bathwildcats.org and click on the registration /open enrollment link
- Create your parent account in FinalForms: https://bath-oh.finalforms.com/
- Upload the required documents into your FinalForms parent account (or bring to the Elementary Office)
- child’s birth certificate (which includes the mother’s maiden name)
- immunization information
- three documents providing proof of residence in the Bath Local School District.
- custody papers (if applicable, including all pages of the document and the judge or magistrate’s signature)
- All of these items must be submitted to the elementary office before kindergarten screening takes place. A screening date and time will be scheduled for your student once the above required items are submitted.
